Why Does Stress Affect Your Skin? Understanding the Skin–Nervous System Connection

Your skin is listening to your nervous system.


If you’ve ever noticed your skin becoming redder, more reactive, itchy, dry, inflamed, or unpredictable during a stressful period, you’re not imagining the connection. Stress and skin are deeply connected.


The nervous system, hormones, immune system, and skin are constantly communicating with one another. And when your body spends too much time in a state of stress, that conversation can begin to show up on the surface.


This is one of the reasons I look beyond skincare when working with sensitive or reactive skin at Hälsa Fusion.


Because sometimes your skin is asking for a different kind of support.


First, what actually happens when we’re stressed?


We often talk about “stress” as though it’s an emotion, but physiologically, stress is a whole-body response.


When your brain perceives a threat—whether that threat is physical, emotional, environmental, or simply the accumulation of too much demand—it activates your stress-response systems.


One of these is the hypothalamic-pituitary-adrenal axis, or HPA axis.


Think of the HPA axis as a communication pathway between your brain, pituitary gland, and adrenal glands. The hypothalamus sends a signal. The pituitary responds by releasing ACTH. ACTH travels through the bloodstream to the adrenal glands, where it signals the production of cortisol.


Cortisol is a steroid hormone made from cholesterol, and it plays an important role in helping your body respond to stress, regulate energy availability, influence immune activity, and maintain balance.


The goal isn't to eliminate cortisol.


You need cortisol.


The problem is what can happen when the stress response is repeatedly activated and your body doesn't get enough opportunity to return to a regulated state. Research has shown that psychological stress activates the HPA axis and can influence skin barrier function, hydration, inflammation, immune activity, and healing.


Your body was designed to respond to danger


Imagine you're walking through the woods and suddenly hear something moving behind you. Your body doesn't stop to ask: “Should I be worried about my skin barrier right now?”. It prepares you to survive. Your sympathetic nervous system activates—the classic fight-or-flight response:


Your heart rate increases, your breathing changes, your pupils dilate, allowing more light into the eye and helping you take in your surroundings. Blood flow is shifted toward systems that are important for immediate action, while blood vessels in the skin can constrict. Your muscles receive more support. Digestion becomes less of a priority.

Your attention becomes focused on detecting and responding to potential threats.


This is an incredibly intelligent response when you're actually being chased by something. The problem is that your nervous system doesn't only respond to physical danger.

It can respond to an email, relationship stress, financial pressure, lack of sleep, illness, etc.


So what does this have to do with your skin?


A lot.


Your skin isn't separate from your nervous system. It is highly innervated, meaning it contains an extensive network of nerve fibres, and it communicates with the immune and endocrine systems. In fact, the skin has its own local stress-response machinery that shares many of the same signalling molecules involved in the HPA axis.


This is part of what researchers increasingly describe as the brain–skin axis or skin–nervous system connection. And this connection helps explain why stress can change the way your skin behaves.


1. Stress can weaken the skin barrier


Your skin barrier is one of your body's first lines of defence.

It helps keep moisture in and unwanted substances, irritants, and microorganisms out.

During prolonged psychological stress, increased stress-hormone signalling can interfere with the production of important epidermal lipids and structural proteins.


The result can be increased water loss, reduced hydration, and a less resilient skin barrier.

And when the barrier is compromised, skin may become much more sensitive to things it previously tolerated.


So your cleanser suddenly feels too harsh, or your usual moisturizer stings, or a products you've used for years suddenly seem to cause redness. Basically, your skin feels tight, hot, itchy, or uncomfortable.


This is one reason I don't automatically interpret sensitive skin as simply “having sensitive skin.”


Sometimes the skin has become more reactive because its protective systems are under strain.


2. Stress changes immune activity


Your immune system and nervous system are constantly communicating.

During stress, hormones and neurotransmitters can alter immune-cell behaviour and inflammatory signalling in the skin. Stress has been associated with changes in both innate and adaptive immune responses and can aggravate inflammatory skin conditions.


This doesn't mean stress is the sole cause of a skin condition- skin is complex.

Genetics, hormones, environment, nutrition, medications, skincare, sleep, allergens, microbiome changes, and many other factors can all play a role. But stress can become an important piece of the puzzle.


3. Stress can contribute to neurogenic inflammation


This is one of the most fascinating parts of the skin–nervous system relationship.

Your nerves don't simply carry messages to your brain. They also communicate locally with the tissues around them.


During stress, nerve fibres can release signalling molecules called neuropeptides, including substances such as Substance P. These signals can interact with immune cells, blood vessels, and other cells within the skin.


One important player is the mast cell.


Mast cells sit close to nerves and blood vessels and are involved in immune defence and inflammatory responses. Stress-related signalling can influence mast-cell activity, which may contribute to itching, redness, inflammation, and changes in barrier function.

This is one pathway through which what is happening in your nervous system can become visible on your skin.


And it's why I pay attention when someone tells me:

“My skin flares when I'm stressed.”

4. Your skin can become hyper-reactive


When the barrier is compromised and inflammatory signalling is elevated, your skin may become less tolerant of its environment:


  • Heat.

  • Cold.

  • Wind.

  • Exercise.

  • Certain ingredients.

  • Hormonal changes.

  • Poor sleep.

  • Emotional stress.


This is where stress and sensitive skin can become a frustrating cycle.

Stress affects the barrier and the barrier becomes less resilient.

The skin becomes more reactive, and you start to see redness or inflammation.

You become worried about your skin so you start changing products.

You watch your skin more closely, and now your nervous system has another thing to worry about.


The cycle continues.

And this is where I see the skin differently


When I work with someone experiencing reactive skin, I'm not only looking at what is happening on the skin.

I'm interested in the landscape around it.


  • How are you sleeping?

  • What does your stress look like?

  • Are you constantly “on”?

  • Are you eating consistently?

  • What happens to your skin around your cycle?

  • Has your digestion changed?

  • What products are you using?

  • Has something significant changed in your life?

  • Does your skin flare when you're emotionally overwhelmed?

  • Do you notice heat, flushing, itching, tightness, or redness when you're under pressure?


These questions acknowledge that your brain, nervous system, immune system, hormones, and skin are biologically connected. The research around the brain–skin axis continues to evolve, but the relationship between psychological stress, skin barrier function, inflammation, and immune signalling is well established.


So, what can you actually do?


The answer isn't simply:

“Relax.”


If you're already overwhelmed, being told to relax can be incredibly unhelpful.

Instead, think about giving your nervous system more consistent signals of safety.


Start with the basics.

Slow your breathing.

Longer, slower exhalations can help shift your body away from a heightened stress response. Even a few minutes of intentional breathing can become a simple daily practice.

Create moments of stillness.

Meditation, guided imagery, gentle yoga, or simply sitting quietly without stimulation can give your nervous system a break. Mind–body approaches have been studied as supportive strategies in stress-related dermatology.

Prioritize sleep.

Your skin is doing important repair work while you sleep, and poor sleep and stress can compound one another.

Move gently.

Walking, stretching, restorative yoga, or another form of movement that feels supportive rather than punishing can help you reconnect with your body.

Simplify your skincare.

When your skin is reactive, more isn't always better. Reducing unnecessary stimulation and focusing on barrier-supportive care can give the skin some breathing room.


Your skin isn't failing you


This is perhaps the most important thing I want you to take away.


If your skin seems unpredictable, reactive, or suddenly different, it doesn't necessarily mean you're doing something wrong. Your skin is responding to information and sometimes that information is coming from the environment. Sometimes it's hormonal, your complex skincare routine, your gut, your sleep, your lifestyle, or your immune system.


And sometimes, your nervous system is part of the conversation.


The skin is not simply a surface to manage. It is living, communicating tissue—and it is constantly responding to what is happening within you and around you.


This is why we look at the whole picture.
